The right hand uses the mouse for aiming as normal, but with the left hand on an X55 throttle to control the ships engines (makes sense, right), and use the throttle handle buttons to do things like switch ammunition types, launch torpedoes and aircraft, etc.

Since 3.18.2 I've had no issues using both in-game but as pf 3.21 the flight stick's inputs aren't registering. I've tried importing the control profiles for the HOTAS (which worked) in the past with no success in getting the stick to work. The throttle works fine and both the throttle and stick work in DCS and War Thunder.Then set 1 binding for left click and one for right on your stick or hotas.
